Release And Indemnity Agreement

The registrant has permission to participate in Valley Tennis Club's (the "Club") programs. I have provided all accurate information and have not falsified any records including, but not limited to, age and medication requirements. I understand there are inherent and other risks of injury involved in sporting and recreational activities. I voluntarily accept these risks. I agree to RELEASE FROM LIABILITY and to INDEMNIFY AND HOLD HARMLESS Club, its agents and Directors from all losses or claims for damages or injury for which me or my child may be liable to any other person, as a result of these activities, whether the claim is based on negligence or any other legal theory.

Cancellations & Refunds

Cancellations or changes to program registrations must be requested at least two weeks prior to the first day of the program for a refund. All refunds will be subject to an administration fee per registration. All refunds are paid by cheque. Without sufficient notice refunds will only be considered if extenuating circumstances apply. All refunds must be requested by email and are to be arranged through our Administrative Manager only.
